Hospitals Can't Admit Patients In ICU If They Or Family Refuse: New Guidelines

Hospitals cannot admit critically ill patients in the ICU in case of refusal by them and relatives

New Delhi:

Hospitals cannot admit critically ill patients in the intensive care unit in case of refusal by them and their relatives, the Union Health Ministry has said in its recent guidelines on ICU admissions.

The guidelines compiled by 24 experts further recommended that when no further treatment is possible or available in a disease or in terminally ill patients if the continuation of therapy is not going make an impact on the outcome, especially survival, then keeping in ICU is futile care.

Further, anyone with a living will or advanced directive against ICU care should not be admitted to ICU.

Besides, low priority criteria in case of pandemic or disaster situation, where there is resource limitation, should be taken into account for keeping a patient in the ICU.

Criteria for admitting a patient to ICU should be based on organ failure and need for organ support or in anticipation of deterioration in the medical condition, the guidelines stated.

Altered level of consciousness of recent onset, hemodynamic instability, need for respiratory support, patients with acute illness requiring intensive monitoring and/or organ support or any medical condition or disease with anticipation of deterioration have been listed as criteria for ICU admission.

Patients who have experienced any major intraoperative complication like cardiovascular or respiratory instability or have undergone major surgery also feature among the criteria.

“The following critically ill patients should not be admitted to ICU — patient’s or next of kin’s informed refusal to be admitted in ICU, any disease with a treatment limitation plan, anyone with a living will or advanced directive against ICU care, terminally ill patients with a medical judgement of futility and low priority criteria in case of pandemic or disaster situation where there is resource limitation (e.g. bed, workforce, equipment),” the guidelines stated.

Return of physiological aberrations to near normal or baseline statu, reasonable resolution and stability of the acute illness that necessitated ICU admission, patient/family agreeing for ICU discharge for a treatment-limiting decision or palliative care have been mentioned in the ICU discharge criteria.

According to the guidelines, blood pressure, pulse rate, respiratory rate, breathing pattern, heart rate, oxygen saturation, urine output and neurological status among other parameters should be monitored in a patient awaiting an ICU bed.

Indictment reveals MKO had links to French spy agency

At the beginning of the fourth session of the trial of the recently detained MKO terror group members, chaired by judge Dehqani in the 11th branch of the criminal court of Tehran province, the prosecutor’s representative said that before reading the indictment, some pieces of information would be read out to enlighten the public opinion.

He pointed out that documents have revealed the MKO’s ringleader Masoud Rajavi’s connection with France and the role of the French intelligence service in the explosion at the headquarters of the Islamic Republican Party (IRP) in Tehran in June 1981.

After the armed action of the terrorist group earlier in June 1981, Rajavi and several members of the MKO discussed whether they should declare that the group had entered the military phase and finally decided to make that announcement, he said.

Afterward, he continued, Rajavi proposed to blow up the office of the IRP and the group decided to make preparations for the move.

Then Rajavi traveled to France, and the bombing operation occurred after his trip, he further explained, adding that Abrishamchi, Bani Sadr, Rajavi and several others then hijacked a plane to flee from Iran to France.

The high-profile trial of the ringleaders and members of the MKO kicked off in Tehran on December 12.

According to the criminal court, some 104 members of the terrorist group as well as the MKO group are sued in the trial for committing inhumane crimes.

The MKO has carried out numerous terrorist attacks in Iran against ordinary people and officials since the victory of the Islamic Revolution in 1979, killing thousands of people. Some estimates suggest that the terrorist group has killed over 17,000 people in Iran.

Israel military sinks into ‘mud of Gaza’, fails to achieve 'goals': Israeli general

An Israeli general has warned that the regime has failed to meet its “unrealistic” task of “destroying Hamas” and is sinking into the “mud of Gaza.”

In an opinion piece run by the Ma’ariv newspaper, Yitzhak Barik said on Monday that Israel is caught in a “vicious circle” and slammed the regime for ignoring the “facts on the ground” and “living in a simulated reality.”

“As time goes by, we are further and further from achieving the goals of the war: the eradication of Hamas and the release of the [captives], and we are sinking more and more into the mud of Gaza,” Barik noted.

He said he believed that Israel has no intention of entering Rafah in southern Gaza, where Hamas exercises complete control because it is “terribly crowded” due to Israel’s ever-expanding evacuation orders which have displaced over 2 million Gazans.

“It is therefore impossible to attack these camps, and in a word, it is impossible to eradicate Hamas there whose soldiers are mixed in with the refugees,” he added.

According to Barik Israel’s failure to eradicate Hamas rule in Rafah and failure to take control of the tunnels under it means that it has not met the main task it set for the war, to “overthrow Hamas rule.”

Therefore, he said, the continuation of the fighting does not add to the achievement of the goals of the war.

“The opposite is true, this fighting brings us heavy losses every day: from explosives and traps that the [resistance fighters] plant for us and anti-tank missiles that shoot at us,” he added.

Barik said that the Israeli regime “must reconsider” the situation in war.

“Some of the ministers and members of the Knesset also understand that this is an unrealistic goal at this stage,” he said, adding, “but are afraid to voice their opinion lest they be accused of harming the fighting spirit of the fighters and the … goal of destroying Hamas.”

“A vicious circle is thus created from which there is no way out,” he said.

He warned that in case Israel insists on continuing the war or seizing control of Gaza, it will “lose the support of the world in general and the support of the USA in the war.”

He added that the “radical approach” will also result in a “war of attrition” with Lebanon’s Hezbollah which could spill over into the region.

Since the beginning of the war on Gaza on October 7, nearly 22,000 Palestinians have been killed in the Israeli aggression. Palestinians in Gaza are forced to flee on a near-daily basis towards the south as Israel’s intensified assault continues into 2024.

Israel's plan to displace Gazans proves regime's genocide

In a statement issued on Monday, the movement denounced “the remarks made by the officials of the Nazi government in the Israeli entity regarding the displacement of our people in the Gaza Strip and the search for countries to deport them to.”

Such statements, the movement added, “are ongoing war crimes that confirm the Zionist aggression on the Gaza Strip is a war of genocide against the existence of the Palestinian people in their homeland.”

Commenting on their remarks, the Islamic Jihad said silence in the face of the Israeli regime’s plan to displace the people of Gaza “implies implicit encouragement for the entity to continue its criminal agenda.”

Such agenda “will not be limited to Gaza alone,” the movement warned, adding that the plan threatens “Arab national security” and poses a “definite danger…to the stability of all countries of the region.”

Concluding the statement, the Resistance movement held the United States responsible for emboldening the Israeli regime to implement such plans against Palestinians “through the invention of the term ‘voluntary displacement’.”

The Islamic Jihad said the US is “giving the [Zionist] entity the time and weapons necessary to destroy all aspects of life in Gaza, so that displacement becomes the only option” for its people.

The Islamic Jihad, however, vowed that Palestinian Resistance groups will not hesitate to stand up to the US-backed Israeli onslaught.

“We affirm that we will continue to confront this war of extermination and displacement, and we rely on the awareness of our people and the bravery of the combatants in the field to thwart these criminal plans,” the movement said.

The Israeli regime launched its war on Gaza on October 7, 2023, following an operation by the territory’s Resistance movements, dubbed Operation al-Aqsa Storm. Close to 22,000 people, most of them women and children, have been killed in the regime’s military aggression so far.

Throughout the war, the regime’s officials have been floating a plan, that requires forced displacement of Palestinians residing in Gaza to other countries.

"Surrendered Meekly…": Ex-India Coach's Brutal Take On 1st SA Test Loss

Former bowling coach Bharat Arun said India “surrendered meekly” against South Africa.© AFP

India were outplayed by South Africa in the first Test of a two-match series last week in Centurion. The Proteas took a 1-0 in the series, courtesy of an innings and 32-run win at the SuperSport Park. With India eyeing a redemption in the series decider in Cape Town, a venue where they have never won a Test, former bowling coach Bharat Arun shared his views on the humiliating defeat in the series-opener last week, saying that the visitors “surrendered meekly” against South Africa.

“Well, to see India come back and [KL] Rahul get that century was quite happening. And there was quite a lot of help in the wicket for the bowlers. So we had a very, very positive feeling about the whole thing. And we thought that we could clearly make a Test match out of this. I think winning or losing a Test match is okay. But I think the way we lost the last Test – we surrendered meekly in the second innings – is not a good feeling,” Arun told RevSportz in an interview.

On being asked if India can make a comeback and draw the series at Newlands, Arun feels Rohit and his men are strong enough to do so. To back his claim, Arun recalled how India won the series in Australia during thier last trip, having lost the first Test after being bowled out for 36.

“Indians are strong enough. They are resilient enough to be able to come back after this Test match. We did come back after being 36 all out in Australia. So losing a Test match is, I don’t think it’s a very, very big deal. But I think how you come back from there is even more important because that shows the character, their resilience, and things like that,” he added.

India have never beaten South Africa in six previous meeting at the Newlands in Cape Town. They have played six Tests at the venue, losing four and drawing two.

In fact, India have only ever won four Tests in the Rainbow Nation. Two of those victories have come at Johannesburg’s Wanderers Stadium (2006 and 2018), including their first-ever Test win in South Africa under Rahul Dravid‘s captaincy 17 years ago.
